WEDNESDAY 21ST SEPTEMBER, 2016  -  UNFORTUNATELY WE HAVE HAD TO CANCEL THE MORNING WALK-IN AND THE BOOK ON THE DAY FOR THE AFTERNOON.  THERE WILL NOW ONLY BE A WALK-IN SURGERY IN THE AFTERNOON BETWEEN 2-4PM. WE APOLOGISE FOR THE INCONVENIENCE.

WE WILL ALSO BE CLOSED ON TUESDAY 20TH AND THURSDAY 22ND SEPTEMBER DURING THE LUNCHTIME PERIOD 12.15-2PM FOR STAFF TRAINING. WE APOLIGES FOR ANY INCONVENIENCE.

PLEASE NOT WE WILL BE CHANGING OUR CONSULTING TIMES FROM MONDAY 6TH JUNE,2016. PLEASE REFER TO THE CONSULTING TAB FOR NEW TIMETABLE OR ASK FOR A COPY AT RECEPTION.

Please note that Dr Balaie is now on Maternity Leave - at present her sessions will be covered by Dr John Hutchings and our Nurse Practioner Julia Osborn and a locum GP.

 

 

Important Information about your Health Records

NHS England are now able to automatically extract personally identifiable information about you from our computer system, using its powers under the Health and Social Care Act 2012. At the end of January 2014 all households in England will receive an information leaflet called "Better information means better care". Please read the leaflet carefully. If you have not received the leaflet you can read it here.

If you do not want your details to be extracted, complete one of the opt out forms which can be obtained from Reception or downloaded from the Care Data page. Please note Patients who have opted out of the Summary Care Record (SCR) will still need to complete an opt out form.

 

 

 

Welcome to Durdells Avenue Surgery Online

Durdell Avenue SurgeryThe doctors and staff at Durdells Avenue surgery in Kinson, Bounemouth are proud to offer the highest standard of patient-centred healthcare.  We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, childhood vaccinations, well-person check-ups.

In addition to everything you need to know about the practice you will also find a wealth of health-related information in the menu on the right hand side. Please have a look around and do send us some feedback if you like.

The Surgery Building

The surgery is a purpose-built two storey building. No steps have to be negotiated to enter it, and most doors and one toilet easily admit a wheelchair.  There is ample car park space attached to The Surgery.


If you wish to participate in the Patient Representative Group please contact the surgery.

Our practice now assigns a named GP to all our patients. If you would like to know which doctor has been allocated to you please ask at reception.  Please continue to book appointments in the same way and you can still continue to see any available doctor at the Practice.

Online Services

THE WAY YOU ORDER YOUR PRESCRIPTIONS ONLINE IS CHANGING.

Our current service will cease on the 1st August, 2014.

In order to use the new online service you will need to register at reception with photo ID.

In future our new online service will also include a facility to view and cancel your appointments. You will also be able to book appointments for certain clinics.

So don't delay - register today!
